Pizza Order: Fall Semester, 2017

Pizza days are Tuesday. This order is for 11 weeks in the 2017 fall semester, beginning 9/26/2017 and ending 12/19/2017. Note there is no delivery on 10/31/2017.

All pizzas are provided by Cosmos Pizza. The following selections will be provided:

  • Cheese
  • Pepperoni
  • Gluten Free Cheese

Price

The cost per slice is $3.00.  The cost for 11 weeks of pizza:

  • 1 slice: $33.00
  • 2 slices: $66.00
  • 3 slices: $99.00
  • 4 slices: $132.00

Upon submission of the form, you will be directed to a payment page.  Payment may be done via Paypal using either your Paypal account or any major credit card.  Alternatively you may pay by check — after submitting the below form, please make check out to SMWS and turn in to the office with a note saying the name of the student(s) receiving the pizza.  PLEASE SUBMIT ONE FORM FOR EACH PERSON — do not include siblings on one order.  Thank you!!
